Skull tattoo printed in black on a grey cover. Lou Reed lead guitar/Warhol credits, There She Comes Now. 

Several editions exist: 
green vinyl, yellow vinyl with a dash of red, purple clear swirl and picture-disc (this). 

According to Modern Records former owner, the actual quantities pressed for this edition would be about 300 picture disc and much less than 1000 coloured vinyl (maybe 300-500).
